1. COP MEMBERS ONLY (Must be an active member for at least 6 months to qualify) 2. If assistance has been received within last 12 months, please do not submit form for assistance. (assistance is allowed only once per year) 3. Please allow 5-7 business days to process a benevolent form. If assistance is needed in less than 24-48 hours, please seek other options for assistance. 4. Maximum assistance for members $300 (special hardship will be decided by benevolent team if more than $300) 5. Before a second request is granted, you must take a financial literacy class by appointed COP staff. 6. After assistance application is submitted, applicant will be notified that application has been received.
